Policies:

I charge $130.00 per session.  Sessions are typically scheduled for 60 minutes.


I am currently a preferred provider with PacificSource, Regence BlueCrossBlueShield and First Choice Health.


If I am billing your insurance and your insurance has an out-of-network provider provision, you will have a co-pay and/or mental health deductible.  Please contact your provider before your first session, to clarify this.  If you have not yet met your deductible, you will be expected to pay my full hourly rate until this has been met.

 

Please bring your insurance card with you to your first session.  This will help me in completing the insurance billing.  Know that when I submit an invoice to the insurance company, it will include a diagnosis, which will be a permanent part of your insurance record.  If you have concerns about this, do not hesitate to talk with me about this.

 

If you need to cancel an appointment, I require a 24 hour notice.  Without such notification, you will be charged $75, half my hourly rate.  If I receive no cancellation and you don't show for your appointment, you will be charged the full $130.  If you are late for an appointment, we will use the remaining time scheduled.

 

I am happy to communicate with you over the phone and through email if necessary, though I would expect that these interactions would be limited in duration.  I would need to charge you at my hourly rate for extended and ongoing phone and email interactions.  The majority of our work together will happen in session.

 

Your Rights as a Client:

As a client of an Oregon Licensee, you have the following rights:

To expect that the licensee has met the minimal qualification of training and experience required by state law;

To examine public records maintained by the Board and to have the Board confirm credentials of the licensee;

To obtain a copy of the Code of Ethics;

To report complaints to the Board;

To be informed of the cost of professional services before receiving the services;

To be assured of privacy and confidentiality while receiving services as defined by rule and law, including the following exceptions:

  1)  Reporting suspected child abuse;

  2)  Reporting imminent danger to client or others;

  3)  Reporting information required in court proceedings or by client's insurance company, or other relevant agencies;

  4)  Providing information concerning licensee case consultation or supervision;

  5)  Defending claims brought by client against licensee;

To be free from being the object of discrimination on the basis of race, religion, gender, or other unlawful category while receiving services.
